Enhance your provider’s encounters with an accurate Pre-visit note. The Clinical Support Agent will contact the patient over the phone (mostly complex with chronic conditions), 1 to 3 days prior to the appointment with the provider and create a draft for the patient visit note, documenting the chief complaint, HPI, ROS, and LAB, Imaging or specialist notes that are pertinent for the visit. The CSA will also recommend ICD10 diagnoses including diagnosis Risk Assessment factor value and suggest missing gaps to improve the patient’s Star Measure. During the office visit, the provider will be able to read, copy, edit, or paste into the note. This minimizes patient care time, improves efficiency, and most importantly, patient satisfaction.
- Since the CSA has completed a medical education, they understand the meaning of the medical philosophy, cannot give medical advice over the phone but is highly proficient in writing and speaking English to perform the following functions:
- Document virtual vitals and notify providers of any urgent matter
- Communicate with your providers, report findings, as well as carry on the providers orders and document them in the patient’s chart
- Talk to patients, either inbound or outbound, and document the encounter in the medical chart
- Identify urgent medical conditions and immediately notify the provider
- Knowledgeable in case management and coordinates the patient’s transition of care (TOC), referrals, hospital record and specialist reports follow-ups, and any other exams such as labs or imagining necessary for the coordination of care.
- Reconcile medications when speaking to the patient on the phone and carry out orders to refill medications approved on standing orders, or contact providers for any changes
- Explain and educate the patient medically
